Grant Applicants

The proposal must a contain an organizational cover sheet, a description of the project, its purpose, and its relation to other activities of the organization. The following factors are among those considered in our examination:

Ø Proposal (excluding attachments) should contain no more than six (6) pages;

Ø The NY/NJ area Common Application Form Grant Proposal  format may be used ;

Ø    Evidence of need of the services to be provided;

Ø     Organizational resources (staff, facilities, finances) to meet the need;

Ø Evidence of local support; and

Ø   Plans for the organization’s evaluation of the project.

Please include the following attachments:

1. A copy of the IRS determination letter of the non-profit organization, a 501(c) (3) ;

2. An audited financial statement or 990 Form for the most recent year available;

3. List of Board Members and their affiliations;

4. A project or capital budget; and

5. The current year’s organization budget.

Proposals will not be accepted by fax or e-mail.

Progress reports and a final accounting of the use of grant funds are required for all grant recipients. In most cases, there may be a follow-up site visit by a Foundation representative after the grant is awarded.
